Pre-K Registration

Documents needed:

• Birth certificate
• 2 current proofs of residency verifying 911 address dated within the last 30 days
• Social security card 
• Immunization record (Form 3231, health record (Form 3300)
• Category 1 documentation if applicable (CAPS, CACFP, SNAP, SSI, Medicaid, TANF, Foster Care)

Please book an appointment through the online calendar or by calling 706-946-2004. When you arrive, you will fill out the Georgia Residency Affidavit form. It must be notarized. A notary will be available when you come to your registration appointment.

• Have the following information available for all students in your household:
  • Student's FULL Legal Name (First, Middle, Last - as it appears on the birth certificate)
  • Student's Date of Birth
  • Student's FULL Social Security Number (optional)
• Write down your application number for reference
• Parents should have a valid email address
• Be sure to check your spam for the confirmation email

   

Please note that you can upload the required documentation while completing the online application. If you are unable to upload the required documentation, please bring them with you to your appointment.
