Registrar's Office

You will be asked to upload the documents listed below electronically (scanned copy, picture via phone, etc.).  If you do not have all of the forms, we ask that you go ahead and complete the registration process.

Documents Needed:

(Take pictures of all documents before you begin)

1. Two (2) Current Proofs of Residence (dated within the last 30 days, i.e. utility bill, paperwork, etc. verifying your 911 address; PO Box is not a proof of residency)

2. Birth certificate 

3. Social Security card 

4. Current Georgia Immunization Record (Form 3231) and/or waiver form

5. Georgia Health Form (Form 3300- Eye, Ear, Dental examination record) *Examinations are available at the Fannin County Health Department, 706-632-3023

Please book an appointment through the online calendar or by calling 706-946-2004. When you arrive, you will fill out the Georgia Residency Affidavit form. It must be notarized. A notary will be available when you come to your registration appointment. 

• Have the following information available for all students in your household:

  • Student's FULL Legal Name (First, Middle, Last - as it appears on the birth certificate)

  • Student's Date of Birth

  • Student's FULL Social Security Number (optional)

• Write down your application number for reference

• Parents should have a valid email address

• Be sure to check your spam for the confirmation email

Please note that you can upload the required documentation while completing the online application. If you are unable to upload the required documentation, please bring them with you to your appointment.

Also helpful in making your child’s transition smooth are copies of your child’s last report card, transcript, and the withdrawal form from the previous school. We will be glad to make copies of these documents for our records. To enroll a child in school, you must be either the parent or legal guardian of the child.
